Before being accepted as a participant in the course, you must read, understand and agree to the terms of the Golden Harmony Kung-Fu Waiver.

This program requires that all participants engage in physical contact with instructors and other participants in order to practice and develop defensive positions, moves, and tactics. The participants may therefore be exposed to personal injury. As physical activity is required, all participants must have a physician confirm that they are in good physical condition, free of any communicable blood diseases, and are physically and mentally capable of participation in this program. I the undersigned confirm that I have read this waiver in its entirety, agree with the terms and hereby assume all risks of injury or damage to me, my property or my dependants however caused by my participation in this program and agree that I will obtain insurance coverage with respect to these risks or assume the risks personally. I further agree not to bring any actions or claims against the program instructors, participants, Golden Harmony Kung Fu Inc., Tai-Mantis Kung Fu Association, Fukien Kuntao Systems, and the owners of the building where the program is held, or their respective agents, officers or employees in respect to injury or damage to me, my property or my dependants however caused. I waive all rights to royalties or other gains from the use of photographs, video recordings, etc., that may be taken and used by the management/instructor(s).
